Vertical maxillary hyperplasia (VME)

The patient with vertical maxillary hyperplasia usually will require a Le Fort I maxillary osteotomy with superior repositioning. The maxilla is impacted superiorly to achieve an ideal vertical tooth to lip relationship at repose, 3–4 mm. The maxilla is positioned posteriorly to allow for ideal intercuspation or occlusion and a harmonious maxillo/mandibular relationship in accord with the tooth to lip position and intercuspation with the mandibular dentition. The vertical position of the teeth within the alveolus should minimally be altered with pre-surgical orthodontics. The ideal vertical position of the dentition is best achieved by the surgical repositioning of the maxilla and dentition as one unit.
